IPROTOCOLOS: a tool for integrating clinical practice guidelines with prescription systems

Resumo: Background: An increasing number of hospitals are developing clinical practice guidelines (CPGs). Effective implementation is important to achieve better healthcare quality. Context: Hospital Moinhos de Vento is a Brazilian private non-profit hospital, with 3,258 registered doctors and 360 beds. The institution have been developing CPGs since 2010; survey with physicians showed these documents were underused. Description of best practice: we developed a tool to simplify assess of our guidelines by health care providers. IPROTOCOLOS consists on a web-based platform, with clinical pathways and recommendations from CPGs, and on a software, that performs the integration with the hospital information system, including prescription and electronic medical records. When the user types a keyword of a specific guideline, a pop-up appear in the bottom of the screen, with the link for the document. The system has a user-friendly interface, which allows professionals easily design clinical pathways, ensuring different CPGs will have the same style, despite the author or the subject. We developed IPROTOCOLOS using MySQL, PHP and JavaScript; currently it is available for Microsoft Windows. We have 126 guidelines in our platform; from September 2014 to April 2015, users assessed CPGs 7,388 times; average session duration was 1:31 minutes. In January 2015, we also implemented IPROTOCOLOS in a medium-sized public Hospital in Brazil. Lessons for guideline developers, adapters, implementers and/or users: IPROTOCOLOS allows easy access to clinical recommendations and monitor guidelines` use. The integration of CPGs with hospital information system is an implementation strategy that appeared to be effective in our setting.
